• To enroll your child for middle or high school in Ipswich, please complete and return the District Enrollment Packet: MIDDLE/HIGH to our Central Office, located at 1 Lord Square in Ipswich.

    A completed enrollment pack MUST include:

    • Residency documentation (as described on the first two pages of the enrollment packet)
    • A copy of your child's immunization record
    • Most recent physical
    • A copy of your child's birth certificate
    • Completed enrollment packet
    • Transcript
    • Copy of IEP and/or 504 Accommodation Plan (if applicable)
